PARTICIPATION REQUIREMENTS FOR SCHOOL YEAR 2007-2008
** ATHLETES WILL NOT BE PERMITTED TO PARTICIPATE UNTIL ALL FORMS HAVE BEEN SUBMITTED AND ARE ON FILE WITH THE ATHLETIC DIRECTOR**

Yellow and green cards are available in the middle school main office

It is our hope that the web site can be a vehicle to relay important messages and CANCELLATIONS, helping to eliminate the number of phone calls being made. If the weather is questionable, please check the web site for practice and game cancellations for that day. These will be posted as soon as a decision has been made.
PHYSICALS (YELLOW CARDS):
All middle school student athletes are required to have a physical examination (yellow) card on file in the athletic directors office. This is required in the 7th grade, and any 8th grader that did not turn in a physical card as a 7th grader. 6th graders are required to provide us with confirmation from a physician that they had had a physical in the past 3 years.
GREEN CARDS:
Members of each athletic team must complete an emergency information card and return it to their respective coach. These cards provide emergency contact information and insurance information.

SPRING SPORTS:
o First practice will be Monday, April 7th. Weather and field conditions permitting.
o Sign-ups will take place on Thursday, March 13th during lunches at the middle school.
