Parents of 6th Grade Athletes
All student-athletes are required to have had a physical within a 3-year window, hence us asking for them to have a physical in the 7th and again in the 10th grades. Since we allow our 6th graders to participate in some of our athletic offerings, we need to ask that you have your child's physician sign off on the fact that your child has had a physical within this 3-year window (not necessarily this year).
Your physician may either email, mail, or fax us a PHYSICIAN-SIGNED message stating that your child has had a physical in the past 3 years and is physically able to participate in athletics at Greely Middle School.
The following statement can be faxed to 829-4819, emailed to Ken_Marks@msad51.org with a scanned signature, or mailed to Ken Marks, Greely Middle School, 351 Tuttle Road, Cumberland, ME 04021:
" I hereby certify that __________________________________ was examined by me on _____________, 20_____ and was found physically fit to engage in athletics during the 2007-2008 school year."
Physician's Signature: ________________________________ Date: __________________________
